Frozen shoulder

Introduction: It is also called as adhesive capsulitis, involves stiffness and pain in the shoulder joint in the earlier stages and in later stages with restricted range of movements. Symptoms: Early Symptoms of Frozen Shoulder are – The full range of symptoms may arise over a period of around two years, but early symptoms include: 1. pain in the shoulder, especially unexpected pain, when no known injury is present 2. difficulty lifting one’s arm above the head 3. difficulty extending one’s arm across the body or reaching behind the back Frozen Shoulder typically develops slowly in three stages. 1. Freezing Stage – Any movement of the shoulder causes pain, and the shoulder’s ability to move becomes limited. This stage lasts from 2 to 9 months 2. Frozen Stage – Pain might lessen during this stage. However, the shoulder becomes stiffer. Using it becomes more difficult. This stage lasts from 4 to 12 months 3. Thawing Stage – The shoulder’s ability to move begins to improve. This stage lasts from 5 to 24 months. For some people, the pain worsens at night, sometimes disrupting sleep. Causes and Risk Factors : 1. Frozen shoulder results from inflammation and tightening of the capsule around the shoulder joint. The Risk Factors include: 2. Medical conditions such as diabetes, overactive or underactive thyroid 3. Immobility or reduced mobility due to arm fracture, stroke, or surgeries such as mastectomy 4. Age: More common among elderly persons 5. Gender: Women are at higher risk of developing frozen shoulder 6. Previous injury Diagnosis : Physical examination to check intensity of pain and range of movement Tests – X–RAY MRI Treatment: 1. Yoga Therapy -Yoga practice helps in relieving pain and stiffness of frozen shoulder, warms up the shoulder joint and frees up the spaces in the shoulder. Yoga opens the shoulder and chest area. This relieves stiffness and permits full and pain-less shoulder movements. 2. Ayurveda Therapies: Oral medicines are advised to reduce pain and inflammation along with external oil based treatments such Patra potalasweda or Elakizhi ( fomentation with medicated leaves bolus) , Pichu (oil padding therapy) and Nasya ( nasal errhines) 3. Stretch the Shoulder: Before beginning shoulder exercises for a frozen shoulder, try to warm up your shoulder by doing a few warm up exercises. This will improve the blood supply in the affected area and avoid future injuries by making your body more comfortable with the motion. The best approach is to extend and warm up the shoulder by applying heat for 10 to 15 minutes, scrubbing down or showering with Epsom salt. 4. Non-Intrusive Treatment: While these activities mentioned above can be easily performed at home, if pain continues and makes it difficult to move around or work ordinarily, see a physiotherapist who can appoint particular activities and can enhance your scope of movement, flexibility and strength. 5. Arthritis can be defined as inflammation or swelling of one or more joints. It also affects the surrounding tissues. Depending on different types of Arthritis, there may be varied symptoms, but they usually include joint pain and stiffness. Commonly seen Arthritis are: Osteoarthritis Rheumatoid Arthritis Osteoarthritis Let’s take a case: “A woman of age 67yrs who was normal three years ago gradually developed pain in both her knee joints. She experienced intense pain while walking, standing, or doing any work. She also experienced sounds (crepitus) while moving her joints. This affected her daily life, and she could not do her routine work. So, she opted for Ayurvedic treatment. She underwent a course of Abhyanga (Massage) with prescribed herbal oils and then underwent Virechana therapy. Gradually, she observed a reduction in pain. She was able to move joints without pain and difficulty.” Now let’s see what Osteoarthritis is: It is the most common type of Arthritis. It usually affects the joints of the hands, hips, and knees. Osteoarthritis is called Sandhigata Vata in Ayurveda, which is one among the Vata Vyadhis. Due to the localisation of morbid or vitiated Vata in the joints, it produces pain. Causes: Excessive exercise, Injury, Suppression of natural urges, Excess consumption of dry foods. Signs and Symptoms: Shula (Pain), Atopa (Crepitus), Shotha (Swelling), Vata purna driti sparsha (Sounds resemble that of when we rub against balloon), Prasarana akunchanayoho pravrutti savedana (Painful movement of joints when extended or flexed). Management: External management: Abhyanga (Massage) using herbal oils, Avagaha, Seka, Local Basthi Internal management: Snehapana, Virechana, Basthi Rheumatoid Arthritis Now, let’s take a case: “A man of age 72yrs who was normal ten years back gradually developed with Indigestion, Anorexia and Weakness. He took medications for indigestion but they didn’t give him complete relief. Slowly he developed pain in his knee joints and fingers. He could not move his joints properly because it caused him much pain. He also developed a fever and generalised body aches. His routine work was affected by this pain. The pain got aggravated when he was exposed to cold weather. So, he was first treated for indigestion by administering a different diet. He started with langhana (Fasting) and then was given tikta katu deepana medicine (Digestion-promoting medicines with a pungent and bitter taste). Lastly, he was given Virechana, one of the Panchakarma treatment. Now, slowly the pain is reducing, and he can work daily easily.” Let’s see what Rheumatoid arthritis is : It is a painful condition which initially affects the gut by producing symptoms like indigestion and anorexia. Later, it affects all tissues, joints, and bones. It is co-related to Amavata in Ayurveda. In this condition, vitiated Vata carries Ama into different body parts and effects. Causes: Viruddha Ahara (Incompatible food), Viruddha Chesta (Unwholesome lifestyle), Nischala (Sedentary lifestyle), Mandagni (Low metabolism) Symptoms: Generalised body ache, Anorexia, Thirst, Weakness, Fever, Indigestion, and Swelling in the affected body parts. Management: Internal management: Langhana (Fasting), Tikta katu deepana (Digestion promoting medicines with a pungent and bitter taste), Virechana, Basti. External management: Abhyanga (Herbal oil massage), Upanaha (Poultice), Swedana (Steam therapy). Gout Let’s take a case: “A man of age 35 years was normal two years ago, and now he gradually developed immense pain in his legs. He could not walk, climb stairs or stand for a long time. Slowly he observed inflammation in his legs and felt stiffness in his joints. Pain aggravated when exposed to cold weather, and he could not work effectively with the pain. He underwent Panchakarma treatment – Vamana (Emesis) and Virechana (Purgation). First, he started with digestion, promoting medicine to treat his indigestion. He was given snehapana to oleate the body and finally took pills for emesis and purgation. After the treatment, he observed a reduction in pain and inflammation.” Now let’s see what Gout is: It is a painful condition in which uric acid crystallises and deposits in the joints, which produces inflammation. This can be co-related to Vataraktha. Here vitiated Vata affects the Raktha dhatu and produces symptoms. Causes: Excessive consumption of salts-sour-pungent-oily and hot foods, indigestion, Sleeping during daytime. This condition of homeostasis brings in a stability in the living organism characterised by normal body temperature, hormone balance and physical and chemical activity in the body. However in the wake of stressful situations in life, the release of cortisol hormone hampers the fine balance and gives way to ill health. Excess of cortisol in the body and the continuation of being in such situations will not only bring down the body’s defence mechanism and immune system but also affect the functioning of various vital organs in the body.   Ayurvedic herbs in management of stress Ayurveda believes in keeping the vital bio-energies or the doshas in balance for the healthy state of body. According to this concept all the herbo-mineral drugs used as tridosha shamaka can be used for the management of stress induced ailments. These naturally occurring plants help in removing the toxic effect of the stress induced hormones in the body. Out of innumerable collections of herbs most common ones are Ashwagandha, Shatavari, Shankhapushpi, Mandukaparni, Brahmi, Tulsi, Aparajita, guduchi, etc. All of these herbs work on “Manovaha Strotas” and are mentioned in samhitas having the properties which benefit the mental health. They are revered as Rasayana or Medya drugs.   Adaptogens Adaptogens are herbs which help in stabilisation of physiological processes and promotion of homeostasis. They contain substances that decrease the sensitivity to stressors through natural ways and help in normalising the body functions. Many herbs from different parts of the world are considered to have adaptogenic effects. For example, ginseng, the most popular Chinese herb, is used widely as an adaptogen. Some researchers claim that these have neuroprotective, anti-fatigue, antidepressant , nootropic and CNS-stimulating activity. Their activity is linked to the Hypothalamic-Pituitary-Adrenal axis and key mediators of stress response such as cortisol and other chemicals in the body. Some of the Ayurvedic herbs which are easily available in raw forms and can be included in our everyday lives as powders, decoctions, drinks or as plain food could hold the key to the fine balance of doshas and check the effect of stress on our body.   Ashwagandha Withania Somnifera It is a nervine tonic and is used in Ayurveda as Rasayana(rejuvenator) and Balya(promotes strength). It is classically used in people with depleted tissues, weak, emancipated, elderly and children with need for nourishment. Ashwagandha is considered as Medya and Smrithivardhaka (improves memory and intellect). Studies have proved its efficacy in relieving stress, anxiety, depression and promoting mental health. It is widely accepted as an herb to treat insomnia. It is a natural antioxidant and hence improves body strength and immunity. Research has proven its usefulness as an excellent immunomodulator. Roots of Ashwagandha are used in powder form for medicinal purposes. 5g of root powder is advised with milk, warm water, honey, ghee for consuming orally. Decoction or herbal tea can be included in your everyday diet. According to Ayurveda it has hot potency, pungent and bitter taste and balances all doshas. It is included under Medya Rasayana, considered as a brain tonic. This perennial succulent plant contains powerful alkaloids such as brahmin, bacopaside, narcoside and others which affect brain activity. Brahmi is used in promotion of brain cell regeneration and has a definitive effect on Alzheimer’s disease. It rejuvenates the mind, enhances memory and concentration. It is most effective in bringing anxiety and hyperactivity of the brain under control. It has a calming effect on the body and mind. It is specifically used in insomnia and other serious neurological disorders. According to Ayurveda it has vata-kapha pacifying properties. Later on we can start doing different types of meditation. A progressive habituation allows the mind to remain relaxed during the period of meditation. OM meditation is one of the great and blissful meditation techniques which provides an elevated state of consciousness. The vibrations of OM after chanting will impact the person’s physical, mental and emotional state. As we believe that universal frequency/sound is OM and when we align this to our practice it will greatly help in developing spiritual awareness. Bahiranga yoga is to gain mastery over mind by using indirect techniques and antaranga yoga is to directly handle the mind and gain mastery over it. Bahiranga yoga consists of Yama, Niyama, Asana, Pranayama and Pratyahara. YamaAhimsa (non-violence): is the awareness and practice of non-violence in thought, speech and action. It advocates the practices of compassion, love, understanding, patience, self-love and worthiness.Satya (Truthfulness): Pathanjali describes truthfulness as “To be in harmony with mind, word and action, to conduct speech and mind according to the truth, to express through speech and to retain everything in the intellect that has been seen, understood or heard.” A perfectly truthful person is one who expresses in his speech exactly what he thinks and in the end acts according to it.Asteya (Non-stealing): Is the third constituent of the Yama, which upholds the unauthorized possession of thought, speech and action. In other words, Asteya is just the opposite of covetousness and envy. It advocates the cultivation of a sense of completeness and self-sufficiency in order to progress beyond base cravings.Brahmacarya (Celibacy): The Vedas, Smritis and Puranas, all glorify the fourth constituent of celibacy. Celibacy is believed to be a behavior, which brings man nearer to the divine. This Yama believes in avoiding all sensual pleasures, whether mental, vocal or physical.Aparigraha (Non-Covetousness): The literal meaning of Aparigraha is non-accumulation of worldly objects, caused by covetousness and attachment. The last state of yama is attained when one remains totally detached from sensual pleasures of all kinds and effectively refrains from committing any sort of himsa or violence. Niyamas: The Niyamas are the second constituent of Astanga Yoga, which explains how we interact with ourselves, i.e. our internal world.Saucha or purity: Soucha includes both external as well as internal purity. According to the words of sage Manu, water purifies the body and clears the mind; and the soul is purified by knowledge and austerity. It advocates the practice of intellectual purity, purity of speech and of the body.Santhosha or contentment: Describes one should not desire more than what he has earned by his honest hard work. This state of mind is about maintaining equanimity through all that life offers.Tapas or Austerity: Described in yoga philosophy as the power to withstand thirst and hunger, cold and heat, discomfort of place and postures, and ritual feasts. It also specifies that the perfect man is one who practices both mental as well as physical austerity.Swadhyaya or self education: Swadhyaya consists of scriptural studies. The scripture being the Vedas and Upanishads together with recitation of the Gayatri mantra.Ishvara Pranidhana: Is dedication of all our actions, performed either by intellect, speech or body, to the divine. The results of all such actions are therefore dependent upon the divine through dedication, purification, tranquilization and concentration of the mind. Asana: Asana is to stay in a position firmly with ease. That’s how Patanjali defines it. The purpose of Asana or Yogasana is to use the body to gain health and mastery over the mind. Yogasanas are practiced slowly and the positions are maintained for longer durations. There are three stages in Yogasanas: Sthira, cira and Sukha. Sthira, the first step involves making the asanas more stable. And in this process, there is great effort involved. Cira which is characterized by relaxation. And Sukha is a stage of bliss. Pranayama: “tasmin sati svasa prasvasayoh gativicchedah pranayamah” i.e., to cut the speed of inhalation and exhalation is pranayama.Prana is related to breathing and when one starts controlling the breathing rate, he starts with pranayam.The respiratory system acts voluntarily and as well as involuntarily. When it is voluntary in nature the breathing is done according to one’s wishes. But when one is not conscious of it, breathing goes on automatically at a particular speed. What one does in pranayama is that he uses the voluntary dimensions to reduce the breathing rate and also balance the breathing between the two nostrils. Pratyahara: The importance of Pratyahara is to use the senses to gain mastery over the mind.Pratyahara consists of two words ‘Prati’ and ‘Ahara’ meaning denying the senses of their food ,Controlling the senses from moving towards objects and thereby controlling the mind. Dharana: Dharana state is when the whole mind focuses on a single point. Initially, everyone goes through a phase where there are several umpteen thoughts floating in the mind. But with concentration it can be reduced to one subject with several thoughts and then can be brought to one subject and to a single thought. This is stage of Dharana. Patanjali says, “Desabandhaschittasya dharana”, Which is binding the mind on a single thought. Dhyana: The next step to Dharana is dhyana or meditation. If Dharana is focusing, then Dhyana is de-focusing. Patanjali gives us the concept of dhyana as an effortless Dharana. As opposed to focusing under Dhrana, there is total de-focussing under Dhyana effortlessly. This is the seventh limb of patanjali’s Astanga Yoga. Samadhi: The mind in the grossest form with a host of random thoughts that is brought into a state of dhyana, first by concentration and then Dharana and then by dhyana .From here, one goes to the eight limbs called the Samadhi. Samadhi is a merger. In all our activities there is what is called Trpti-the seer, the seen and seeing i.e. subject, object and the interaction; In meditation, there are three aspects that include who is meditating, the object of meditation and the process of meditation. The most common example is Rheumatoid arthritis. World Autoimmune Arthritis Day was established in 2012 by the International Autoimmune Arthritis Movement (IAAM) and is celebrated each year on 20 May. Lack of awareness leads to delayed diagnosis, compromised quality of life, and elevated healthcare costs. Educating about these diseases, including the fact they can affect many parts of the body (joints, tissues, organs) is necessary to improve the lives of the 450 million worldwide affected. According to international foundation of Auto immune and Anti-inflammatory Arthritis the main goal is to: Educate the public and medical community about key disease features that can lead to early detection, referrals, and diagnosis. Address misunderstandings associated with these diseases. Let’s understand more about arthritis and management using Ayurveda, Naturopathy and Yoga therapy. Types of Autoimmune arthritis Rheumatoid arthritis: It is one of the commonest autoimmune condition which can affect other parts apart from joints. Immune system attacks its own body tissues. The immune cells are found in the lining of the joint and these cells attract other immune cells and together lead to thickening of the joint lining, new blood vessel formation and damage to joints. This is termed inflammation. This makes your joint swell as fluid builds up inside it. Your joints become painful, swollen, and warm to the touch. Which usually causes swelling and pain in the hands, feet, and wrists. Psoriatic arthritis: Psoriatic arthritis is a form of inflammatory arthritis. Psoriatic arthritis can occur in people with a skin condition called psoriasis. Psoriasis causes scaly, patchy areas to build up on the skin. The affected joint areas due to psoriasis can be almost anywhere on the body, including the spine, knees, fingers, toes, or more. Reactive arthritis: It is inflammatory arthritis that manifests after several days to weeks after a gastrointestinal or genitourinary infection. Reactive arthritis occurs in people who have a history of certain bacterial infections, such as Chlamydia, Salmonella, Shigella, or Campylobacter. The disease is recognized by various symptoms in different organs of the body that may or may not appear at the same time. It may come on quickly and severely or more slowly, with sudden remissions or recurrences. Along with pain in the joint and inflammation, mouth ulcer and eye infection, burning micturition is common. Juvenile arthritis: is the most common type of arthritis in children under the age of 16. Some types of juvenile idiopathic arthritis can cause serious complications, such as growth problems, joint damage and eye inflammation. Management through Ayurveda Ayurveda, the ancient traditional system of medicine treats Autoimmune Arthritis with following principles Balancing the Doshas Increasing the digestive and metabolic fire or Agni.
